GeoSentinel® yellow fever cases – France ex Brazil, the Netherlands ex Brazil

Eric Caumes from the Paris (PRS) GeoSentinel® site has today reported a case of yellow fever (YF) in a 42-year old, unvaccinated female tourist and VFR who returned from Brazil. The diagnosis was initially made in Brazil and the patient was seen after return in Paris during convalescence. The patient was ill in Brazil, where she was hospitalized in a local clinic. She presented with YF compatible clinical and biological signs (hepatitis, thrombocytopenia, and neutropenia), positive YF blood PCR, and positive IgM (and negative for dengue fever). Documented yellow fever risk exposure areas: Brumadinho, Minas Gerais – Inhotim Botanical Garden. Full details of this case will be communicated at a later date. ® This is the second case of yellow fever ex-Brazil to be reported to GeoSentinel® sites in Europe. The first GeoSentinel® case was notified by Perry van Genderen, from the Rotterdam (ROT) site, in January this year and was posted on ProMed. This described an unvaccinated, 46-year old male humanitarian worker who worked in a street project to help child drug addicts in Sao Paulo. This male patient recovered and was discharged on January 15th, 2018. A full report is in preparation. Both these cases illustrate the utility of the GeoSentinel® database in confirming that travelers are sentinels for the expanding yellow fever outbreak situation in Brazil particularly in urban areas that were hitherto considered free of infection. The cases also highlight the importance of YF vaccination for travelers to outbreak areas. For those advising travelers in a pre-travel setting, the expanded yellow fever vaccination recommendations for Brazil and strict adherence to day/evening-time mosquito bite protection measures should be observed.
